What do you need to apply for Medicare if you have End Stage Renal Disease (ESRD):

• Get a CMS-2728 form completed by your dialysis center.

• Go to cms.gov (Centers for Medicare and Medicaid Services).

• Download the CMS-43 form and complete it.

• Find your local Social Security office to email copies of the forms to. The CMS-43 form will need a wet signature.
